PRC Annex A, B, C Form 2022 PDF – Details

If you plan to take the licensure exam, you must submit a duly completed Informed Consent (Annex A) and
Health Declaration (Annexure B) form along with Notice of Admission (NOA) and negative RT-PCR test result/quarantine certificate to the Proctor on the day of examination.

Board exams during the COVID-19 pandemic

The following precautionary health and safety guidelines shall be followed at all times during the conduct of the PRC License Examination:

• Wearing face mask and face shield. It is mandatory for the examinees to bring alcohol-based sanitizer as well.
• The examination personnel will be provided with face masks, face shields, and latex gloves.
• Follow physical distancing by maintaining a distance of 1 meter from each other.
• The body temperature will be checked with a thermal scanner before entering the examination premises. People with fever, cold, or cough are not allowed to take the test. Their payment will not be forfeited and can be used in the next license exam.
• Candidates are advised to bring their own food as they will not be allowed to leave the examination hall during break time and lunchtime.
• The examinees shall sanitize before entering the examination hall, after using the rest room and before distribution of the test questionnaire.
